FEES AND INSURANCE

  • My fee is $275 per 45 minute session, and I am out of network with all insurance plans. If you have health insurance with out of network benefits, you may be eligible for partial reimbursement of out of pocket fees. I encourage you to contact your insurance company to learn more about your out-of-network benefits; some plans reimburse up to 80%. Some sample questions to guide your conversation are included below:

    • I am consulting with a licensed psychologist who does not accept insurance, so I would like to understand my out of network benefits. Dr. Karp’s NPI# is 1316425242, and the CPT code that she will most commonly use is 90834-95. Based on that, can you tell me what percentage/dollar amount I can expect to be reimbursed, if any?

    • Do I have a deductible I must first meet? If so, how much is that? (recommended question)

    • What are the UCR (“usual, customary and reasonable”) charges you allow for these services?

    • Is there a maximum amount you will pay for an office visit/in a given year?

    • Do you cap the number of visits in a year?

    • Is there a specific claim form I must use to submit claims and if so, where can I access that?

    • How much time after a visit do I have to submit a claim?

GOOD FAITH ESTIMATE

  • In compliance with the No Surprises Act (in effect as of January 1, 2022) all healthcare providers are required to notify clients of their Federal rights and protections against “surprise billing.”

  • This Act requires that providers notify you of your federally protected rights to receive a notification when services are rendered by an out-of-network provider, if a client is uninsured, or if a client elects not to use their insurance.

  • Additionally, you are entitled to a Good Faith Estimate (GFE) of the cost of services. It is difficult to determine the true length of treatment for mental health care, and each client has a right to decide how long they would like to participate in mental health care. Therefore, below you will find an example GFE for the services typically offered in my practice, and I will collaborate with you on a regular basis to determine how many sessions you may need.

  • The example below may help you have an idea of the financial expectations for starting therapy with Dr. Karp, however this varies based on the session fee, duration and agreed-upon frequency. The GFE is neither binding nor a guarantee that the patient and therapist will work together. The following example of a GFE is for a 12 month period:

    Individual 50 minute session: $275

    Weekly 50 minute session for 52 weeks: $14,300

    Bi-weekly 50 minute session for 52 weeks: $7,150
